What Is a Follow-Up Testing?

After an employee passes their return-to-duty drug test and resumes safety-sensitive work, the DOT requires an ongoing program of unannounced follow-up testing as directed by the Substance Abuse Professional (SAP). Under 49 CFR Part 40, follow-up testing must include at minimum 6 tests in the first 12 months following return to duty, and may continue for up to 60 months total. The tests are unannounced — the employee does not know in advance when they will be selected. SafeTouch Enterprise coordinates the follow-up schedule with your SAP and provides same-day directly observed collections throughout the program.

How many follow-up tests are required?

The Substance Abuse Professional (SAP) determines the minimum number and frequency. DOT regulations require at least 6 unannounced follow-up tests in the first 12 months after return to duty. The SAP may require additional tests — up to 60 months total — based on the employee's history and clinical assessment.

Will the employee ever know in advance when a follow-up test is coming?

No. Follow-up tests are always unannounced. The employee is notified on the day of the test and must report for collection as soon as possible. This unpredictability is the core deterrent effect of the program.

What happens if a follow-up test comes back positive?

A positive follow-up result is treated as a new DOT drug and alcohol violation. The employee is immediately removed from safety-sensitive duties and must restart the SAP evaluation process from the beginning — including a new treatment plan and another return-to-duty test before returning to work.

Who pays for follow-up testing?

Under DOT regulations, the employer pays for all required follow-up testing. Some employers include this in their written drug-free workplace policy. SafeTouch can bill your employer account directly and maintain the testing records required for compliance audits.

Cancellation Policy

Cancellations must be made at least 48 hours in advance to reschedule without a fee. Cancellations or reschedules made within 48 hours of the appointment are subject to a rescheduling fee. No-shows (no call, no appearance) require rebooking before a new appointment can be confirmed. We do not offer refunds on completed tests or services.
